Bachelor of Science in Nursing (BSN)

Standard Application – Please print the application form.  Complete and send to the address at the top of the application with a $50 application fee and your essay.  References may be sent as completed.

Essay - Submit your application with a brief essay.  Your essay should reflect on why you have chosen nursing as your profession.  What are your short-term goals?  What are your long-term goals?  These may include personal, educational and professional goals.  Please include your name on the essay, print it, and submit by mail with your application.

Reference  Form – Please submit three professional references using this reference form.    It is important that your references address your ability to be successful in a baccalaureate nursing program.  Technical references (such as CNA or EMT instructors) and personal references (such as family, friends, and neighbors) are not accepted.

Pre licensure BSN applicants must include at least one from a current or recent college liberal arts/science instructor; a science professor or instructor is preferred.   All three may be from professors or you may include employers.  

RN to BSN applicants may submit employment references only. 

Early Entry High School applicants may use high school teachers and employers. 

Master of Science in Nursing (MSN)

Standard Application – Please print the application form.  Complete the application and the 3 Letters of Recommendation and send to Saint Anthony College of Nursing, 5658 E. State Street, Rockford, IL, 61108, Attn: Graduate Affairs Department with a $50 application fee.

Student-at-Large Application – Please print the application form.  Complete the application and send to Saint Anthony College of Nursing, 5658 E. State Street, Rockford, IL, 61108, Attn: Graduate Affairs Department with a $50 application fee.

Post Master’s Certificate – Please print the application form.  Complete the application and send to Saint Anthony College of Nursing, 5658 E. State Street, Rockford, IL, 61108, Attn: Graduate Affairs Department with a $50 application fee
