Alumni Sponsorship Application – Please print the application form.  Have alumnus complete the sponsorship section on the second page and send completed to the address at the top of the application.  Application fee is waived with alumni sponsorship.

Master of Science in Nursing (MSN)

Standard Application – Please print the application form.  Complete the application and the 3 Letters of Recommendation and send to Saint Anthony College of Nursing, 5658 E. State Street, Rockford, IL, 61108, Attn: Graduate Affairs Department with a $50 application fee.

Student-at-Large Application – Please print the application form.  Complete the application and send to Saint Anthony College of Nursing, 5658 E. State Street, Rockford, IL, 61108, Attn: Graduate Affairs Department with a $50 application fee.
